[ubuntu] Dialog not saving

I'm new to programming in Ubuntu and wish to create a very simple dialog in a spreadsheet.

I create a new dialog (effectively a form) with a button, which when clicked displays a message - "Hello".

This works well and I attempt to save the file. However, when returning to the file the code is saved but NOT the dialog.

Please identify my error.
